That’s important. It’s why the people in Arizona and the people in California can expect the same basic quality of care.

This is also why it’s relatively easy to get licensed in one state after having worked in another.

However, there are some regional variations—both in law and in the scope of responsibility—that warrant consideration. So, how big of a role will the place you work play in your nursing career? Let’s get into it.

Rural Hospital

How Much Does Your State Influence Nursing Licensing Requirements?

To start with, you should note that the vast majority of states will allow you to get your license relatively easily. Pay a fee. Get your license. As long as you have your BSN and have passed the NCLEX, that’s all they will ask of you.

This thanks to the Nurse Licensure Compact. It’s an (almost) nationwide agreement designed to make nurses’ lives easier. Instead of getting recertified anytime you move, you can pay a fee and be ready to roll.

Not every state is “compact.” Those that don’t acknowledge the agreement vary in their requirements. Rarely will they need anything radical from you. The primary difference will be that you will need to submit to a manual review process. The steps will be roughly the same on your end. They will just take longer to complete.

In many cases, the hospital that has hired you will act as a resource to help you navigate the process.

Do Your Responsibilities Change Based on Location?

That’s an interesting question. The answer is far from definitive. There are variations, of course, but they don’t follow a neat, easily defined path.

For example, one might assume that nurses working in suburban or urban hospitals are radically busier than those in rural settings.

This assumption isn’t always correct. While urban areas may serve a larger population, rural hospitals are often the sole healthcare resource for multiple communities.

An urban hospital might serve several neighborhoods within a five-mile radius, while a rural hospital might cover several counties across fifty miles or more. In that way, the day-to-day reality of rural and urban nurses can be surprisingly similar.

You might also assume the type of care provided differs dramatically between the two. To a limited extent, this at least is true. Urban hospitals might treat more incidents of violence.

That said, rural hospitals still see their share of car accidents, farming or factory injuries, and yes, even violence. Rural communities have also been hit just as hard by public health crises like the opioid epidemic.

This, is all to say that rural nurses don’t have things easier than their urban counterparts. In many cases, they are stuck trying to do the same work with fewer resources.

Shortages

Staffing shortages are felt across the country. Unfortunately, though, they tend to hit rural hospitals the hardest. The reason is pretty straightforward: the pool of local candidates is much smaller. A rural area might produce one or two new nurses a year. A city might graduate dozens or even hundreds.

To fill the gap, rural hospitals often try to recruit from outside the area—but that’s easier said than done. What incentive does one have to become an RN in central Missouri, for example?

If you work in one of these settings, expect to feel that shortage in your day-to-day experience.

Ironically, these shortages are often self-perpetuating. The stress of working in an overtaxed hospital is often enough to make even passionate nurses look for new work.

Diversity of Opportunity

It’s not always easy to predict where the best nursing opportunities will be. Every community has the same essential healthcare needs. The real difference lies in which places can afford to staff accordingly.

Nursing is an incredibly diverse profession. A single community might employ dozens of different types of nurses.

Nurse practitioners. Forensic nurses. Gerontology nurses. If you’re aiming for career flexibility or know that you want to specialize eventually, it’s smart to start out in a location that offers room to grow.

Advanced Practice Positions

One of the biggest lifestyle differences in nursing comes into play for advanced practice nurses. In some states, nurse practitioners are granted nearly full autonomy—they can open their own practices, diagnose patients, prescribe medication, and create comprehensive treatment plans without physician oversight. In others, they perform similar work but must do so under the supervision of a doctor. Naturally, that creates two very different professional experiences. Breathwork provides a direct pathway to parts of ourselves that remain inaccessible through conversation alone. When we consciously alter our breathing patterns, something remarkable happens – we bypass the logical, analytical mind and connect directly with the autonomic nervous system.

This matters tremendously for trauma healing because:

  1. Breath directly affects our physiological state. By changing how we breathe, we can shift from a stress response to a relaxation response within minutes.
  2. Conscious breathing brings awareness to sensation. Many trauma survivors have disconnected from bodily sensations as a protective mechanism. Breathwork gently reconnects them with their physical experience.
  3. The breath serves as an anchor to the present moment. Trauma often keeps us trapped in the past. Focusing on your breath pulls you back to the present, where healing happens.

In my practice, I’ve observed that breathwork accesses subconscious material that clients weren’t aware was impacting them. Memories, insights, and emotions that had been suppressed often surface during breathwork sessions, allowing for processing and integration that wasn’t possible before.

The Science: Why Breathwork Creates Immediate Physiological Shifts

The efficacy of breathwork isn’t just anecdotal – it’s backed by neuroscience. Research has shown that controlled breathing practices can:

  • Activate the parasympathetic nervous system (our “rest and digest” mode)
  • Reduce levels of stress hormones like cortisol
  • Increase heart rate variability, an indicator of emotional resilience
  • Release tension held in the diaphragm and other areas where trauma is stored
  • Improve oxygen flow to the brain, enhancing cognitive function

These physiological changes create a foundation for healing that talk therapy alone simply cannot provide. When the body feels safe, the mind can begin to process traumatic material without becoming overwhelmed.

My Journey: From Anxiety to Healing Through Breath

My path to breathwork began in college when I was struggling with debilitating anxiety and an eating disorder. On the surface, I appeared to be a typical student, but internally, I was in constant turmoil.

Traditional therapy helped me understand the roots of my anxiety, but knowledge alone didn’t stop the panic attacks or the persistent feeling that something was wrong. It wasn’t until I stumbled into a breathwork class at a local yoga studio that I experienced a profound shift.

During that first session, I felt a peace I’d been searching for my entire life. The practice allowed me to access and release emotions that I had been carrying for years. For the first time, I felt present in my body rather than at war with it.

This experience was so transformative that it became my life’s work. Over the past 24 years, I’ve witnessed similar breakthroughs with hundreds of clients who had tried numerous therapeutic approaches without success.

A Simple Practice to Try at Home

  2. Reconnecting mind, body, and spirit. Trauma often causes fragmentation – a disconnection between thoughts, emotions, and physical sensations. Breathwork helps restore integration.
  3. Creating new neural pathways. Regular breathwork practice helps the nervous system learn that it’s safe to relax and develop new responses to triggers.
  4. Empowering self-regulation. Unlike approaches that depend on a therapist’s presence, breathwork offers tools that can be used anywhere, at any time.

The result is not just feeling better temporarily, but fundamentally transforming how we respond to stress and challenging emotions. Over time, many find that their baseline state shifts from anxiety and reactivity to greater presence and peace.

Integrating Breathwork with Traditional Approaches

Breathwork isn’t meant to replace traditional trauma treatment approaches – it enhances them. The most effective trauma healing often combines:

  • Talk therapy for cognitive understanding and processing
  • Somatic practices like breathwork for physiological regulation

  • Community support for connection and validation

When these elements work together, healing can occur on all levels – mind, body, and spirit.

We all experience it in different quantities and at various stages of life. I look at my daughter in her 20s trying to manage the weight of education, work experience, and be social and wonder how she does it all. Stress as a young person is expected as you try to make your way in the world. But for an over 60-year-old, I thought this would be the time to live freely in relaxation and bliss.

I am way beyond my mid-life crisis years (one caravan, 5 boyfriends and a minibar in the garden later), but stress seems to creep into my life no matter how hard I try. Is it the hormones? Am I just a natural pessimist born to attract stressful life experiences? I think both play a part, but ultimately the true challenge is how to overcome the stress. In saying this, I make it sound like stress can be defeated. Stress will always be with us, but moving through rumination and finding ways to ground ourselves is what reminds us stress is just a coping mechanism. You will be okay no matter what pressure you face.

Here are just a few consistent stressors in my life, and things I’m seeing more and more of within online conversations:

Stress moves through time

I’ve worked for the best part of the last 40 years in service of the NHS. Was there a good work-life balance? At the time it didn’t feel like it, but I somehow managed to get married, have four babies and make a thousand more memories outside of the workplace. Now I work part-time just to tick over, yet I feel the same level of stress as someone who works full time.

My current colleagues are in the place I was 40 years ago (trying to start a family), except the cost of living is at its highest, households are earning less and people don’t have the money to buy homes. Perhaps I feel a sense of guilt that I had better means to get by and start a family in my day. My partner at the time was a struggling photographer, so I was the primary earner. I still got by, I moved through the stress, and sometimes I think back to those really difficult times when I think I’ve got it bad now. Yet, minor inconveniences can get me down, and only when I take myself out of a situation and gain a clearer head do I realise how worked up I get over nothing!

Financial stress

Earning money is hard, and in today’s economy, it’s even harder to save. I am lucky enough to have built up a safe investment portfolio and retirement fund. But the cost of everything has increased exponentially. I worry not only about my own ability to stretch the funds but also about supporting my children through their financial stressors. Plus, the constant marketing campaigns pushed into our phone screens make us feel like daily spending is nominal.

Social media

The irony in social media is how unsociable it is. We all rely on these social platforms to keep up with others’ lives. What used to be a catch-up over a cup of tea is now coming across an update on Facebook. We are all so disconnected, and with this, comes the inevitable comparisons, and feeling of ‘lack’. Just because Suzanne from my yoga class has bought a new home and is travelling the world, posting about her adventures shouldn’t take away from my life and achievements. However, in times of doubt, it does. The stress of needing to feel adequate to others online is draining. Truthfully, I would quit social media now if I could, but there are people I want to keep in touch with, so the cycle continues.

Stress affected my health

It snuck up on me, as these things do. Poor sleep, stiff joints, nagging headaches, the constant tiredness that you can’t explain away with “just getting older.” My blood pressure crept up. I lost patience with silly things that wouldn’t normally bother me. When you let stress rule the body, it will take whatever space it’s given. I knew if I didn’t start acting differently, no pill or pep talk was going to fix it for me.

How I’m fighting the battle:

I walk everywhere
It’s not just about the exercise, it’s about feeling the fresh air, seeing real faces, hearing real sounds. It pulls me back to now.

Pilates to make me feel young again
I need flexibility more than ever, and there’s something satisfying about holding a plank longer than someone half my age. It reminds me that my body is still capable.

Swimming for the joints
There’s a peace that comes with floating, and knowing that while gravity does its worst, I can still move without pain.

Spending time gardening
Growing something! Even if it’s just a sad-looking tomato plant it grounds me. Literally. Soil under the nails is strangely healing.

Reducing screen time
It’s a work in progress. I’m trying to replace scrolling with something — anything — that actually fills me up rather than drains me.

Reading — use it or lose it
Novels, history books, gardening guides, it doesn’t matter. If I’m not using my brain, I feel it getting rusty. Plus, there’s a comfort in being pulled into someone else’s world for a while. I just had conservatory insulation, so I spend hours in my cosy backroom falling into stories. Any stress of the day disperses when I’m immersed into a book.

Setting boundaries
Learning to say “no” without guilt is a skill I wish I’d picked up decades ago. Now, I protect my time and energy like a dragon guard’s treasure.

Stress isn’t going away, not for me, not for anyone. But learning how to live alongside it, how to loosen its grip when it gets too tight, is the best I can do. And honestly, at 60, I’m learning that’s enough.

Understanding Intersectionality in Practice

Clients never come into a session defined by one label. They carry multiple identities—race, gender, ability, class, sexual orientation, and more—that intersect and shape their experiences. A culturally competent social worker doesn’t look at one part of a client’s identity in isolation. Instead, they ask, “How do these different parts of who you are interact, and how do they impact the support you need?” This approach allows for deeper empathy and more targeted interventions. Intersectionality isn’t a theoretical framework reserved for academia. In practice, it becomes the difference between making someone feel seen or forcing them to fit into a box.

Culturally competent social work isn’t about having all the right answers—it’s about asking better questions, listening with intention, and staying open to learning every single day. It requires commitment, humility, and the courage to face one’s own blind spots. Social workers who practice true cultural competence understand that their growth never stops because people and communities continue to evolve. Moving beyond buzzwords means showing up with respect, building relationships that matter, and being brave enough to challenge the systems that fail those we aim to support. That’s the kind of work that creates real, lasting change.
